From my understanding, these national means are compromised of 3rd year medical students who have takek the test as their last 3rd year shelf (ie. have completed all rotations in 3rd year and prior shelf exams). This isn't the case for all shelfs, though. IM shelf last year had μ=78 and σ=7 which does correlate to 76th percentile, but surgery shelf μ=74 and σ=8, which correlates to 83rd percentile.
